Is there any cloud or other solution for conducting closed surveys with pre-registration?

We need a tool for conducting online surveys in closed focus groups. The scheme is as follows: the user registers in the system (in a specific focus group) and fills out a personal profile; after creating a survey for his focus group, he receives a mailing list inviting him to take the survey; by clicking on the link, he enters the system with his login / password and answers questions; the system saves the responses of each of the participants and provides the ability to display individual responses (or, for example, the dynamics of responses of a particular participant to a number of surveys).
Please tell me some ready-made solution that combines the functionality of a mailing list and a poll generator.

surveymonkey.com and it seems to be the only Russian survey service. But I myself am looking for a replacement for it, ideally to run on my server and conduct surveys within a local network without the Internet.
A unique link and a pool of polled IDs are generated there. They can fill out questionnaires (screening) themselves or one of the employees will do it. I can't say anything about "logging in with your username / password".
If you are ready to understand and finish - Limesurvey.org, a free PHP poll server. Cons - there are minor bugs of various harmfulness, there is no standard sane design template, there is no way to go to a specific issue with little bloodshed, if you ever need to display more than 1 question on 1 screen, and the rest one at a time, you will have to create dozens of groups, 1 per screen. every question.
