Tell CRM suitable for tasks

Looked at many different systems. Some are very complex, some are simple. That's why I may have missed something. Tell me, you need a system that meets the following requirements and capabilities:

1. You can "make" an application form that will go directly from the sites to CRM. It will be divided according to the completed data and tasks.
2. You can correspond with the client using the CRM itself.
3. You can call the client from the system, and a recording of the conversation will be attached.
4. You can receive email from the client directly into the system.
5. Can prepare and send template documents based on customer data and send (merge document and data).

Is there already such a thing? If there is somewhere partially, poke pliz, indicating certain points that are implemented.

amocrm (1 - through integration with wufoo, 2 - there is integration with email like "put a service mailbox in a copy", 3 - through integration with asterisk or onpbx [generally looks mega cool ] , 4 - see p2, configure forwarding of all incoming messages to a service mailbox system box, 5- fail here)
integrations mostly work out of the box and are configured in a few clicks (with an asterisk, a little more complicated).
In general, widgets are an interesting topic that allows you not to support your crutch for various specific things like mailing lists, but simply to use the market leaders.
