See mysql logs. Queries without indexes are possible. Perhaps some thread innodbshnaya whistle flew off. Everything will be in the logs.
UPD. Well, they would write the characteristics of the machine + the average size of the tables + the load on mysql (requests per second), for example, you can see it through mytop.

SHOW PROCESSLIST
Log slow queries - write something like this in the config:
slow-query-log = 1
slow-query-log-file = /var/log/mysql/mysql-slow.log
long_query_time = 1
log -queries-not-using-indexes
