The HDD is not added to the RAID array. What could have happened?

Faced such a problem.
There is a server on which the LSI MegaRAID SAS RAID adapter is installed.
One of the disks began to blink orange and its status changed to Failed. The disk has been replaced.
The old disk was connected to a regular computer, and as it turned out, it was working. It has been formatted. Check SMART, surface and run all the tests that you can.
We decided to insert it again into the RAID, but it still blinks orange there, and MegaCli -PDList -aALLit is not in the command output.
Hitachi HDS725050KLA360 drive with K2AOAB5A firmware.
The rest of the drives of the same model and with the same firmware work fine in the array.
Maybe someone faced such a problem? What could have happened to the disk?
