Ideally, you want the add form to be editable, such as Contact Form 7 (add/remove fields).
I tried LDD Directory Lite and Business Directory Plugin - they are too confusing, cumbersome and with an outdated design + attempts to impose their paid extensions.

On themeforest you can see Directory. I did a similar functionality on it and it does a very good job.
You only need to finish the map a little if there are more than 3000 enterprises in the category, because gMaps crashes with such an influx of data from the database.
