How to store the results of medical examinations?

Who keeps and how the results of their tests, visits to the doctor, sores, etc.? For me, the state medical card. polyclinics are some kind of formal bale, and I rarely saw doctors who used history, they already have limited time for appointments, no one sorts this rubbish from various forms, handwriting, etc.
Maybe there are some systems / services for accounting for such information, or is the storage standard international? At least what systems are used by medical institutions themselves, although their functionality is redundant for the patient, of course ..
