How to set up a task status change in Jira so that a window with Assign appears?

The Lord needs your help. I added different statuses to Workflow (Code Review, Feedback). And I wanted to set up Jira so that when the tax status changes, a window appears with Assign and a comment, so that everything can be done in one action. How to implement it?

This is done very simply. A screen is made (Admin-Screens), the screen is hung on a transition (In Admin-Workflows-Necessary workflow). Validation of fields for filling can also be done in a transition.
