There is a site proteks.h1n.ru , it has an adaptive slider (owl slider) with a form that is also adaptive.
The problem is that when entering data and when automatically scrolling the slides, the user may not have time to enter data into the form and the slide will change to another with a new same form, that is, each slide has a separate form that duplicates the first form.

In general, you need to make only one form and so that it is in the same place as it is now and fixed when changing slides, i.e. the user could enter data regardless of the change of slides, despite the fact that the form still had to be adaptive as it is now.
