How to properly request broadcasting and distribution rights from the author?

I liked the short film, the author did not indicate any license (SS 3.0 for example), I want to broadcast this cartoon on TV in my organization. I understand that permission is needed, but how do I get it? What document will be proof of the transfer of broadcasting rights? What to send him?

The author is not obliged to deal with you. And you don't have to provide contact information anywhere. Even if you talk to him, he does not have to agree to grant you rights. Even for money. His right (unless he sold it, of course).
And how you reach out to the author and convince him to transfer part of the rights to you, it already depends on the specific case. People are different. For example, I don't know if you can offer the author a million dollars or ask the author to pay you extra for broadcasting. Both options have the right to exist, as agreed.
For example, you can do this:
- Dude, I want to broadcast your short film on TV. Can?
- No problem.

To begin with, you do not need an author, but a copyright holder (owner of exclusive rights). It could be a completely different person or organization.
And the proof will be an agreement that specifies the specific transferable rights and their scope, for example, broadcasting in the organization's local network, 10 repetitions.
