How do I withdraw funds from Upwork to Payoneer?

To withdraw funds, I need to fill out the W9 Form, but when I click on the link to this form, I get the W-8BEN form with the "Legal Name of Business" field.
Why is the "Legal Name of Individual" field not showing up?
How to proceed?
