Can a qr code be read incorrectly?

Qr code as hash or not?
Let's say if a fragment of a picture is damaged in some part, can the encoded message change? or how would the hash be violated and the message not be displayed already?

If the code was read and decrypted, it is correct
If not decrypted, then the encrypted data is lost
