Yandex does not see title and description meta tags. What is the problem?

Yandex.Webmaster gives two problems:

Missing title tags
A significant portion of pages do not contain a title tag, or it is filled out incorrectly. This can negatively impact how your site appears in search results.

Description meta tags are missing Many of the site's pages have missing or incorrect description meta tags. This can negatively impact how your site appears in search results.

At the same time, on any page of the site in the head you can find meta data :

Page re-bypass was more than once. The code goes on top of head right after the Yandex.Metrica script. How to solve this problem? Thank you!

Perhaps you should indicate at the very beginning of the page what markup is used or remove itemprop="name"
