Will Windows LTSC be upgraded from 1089 to 21H2 via Windows Update? Or do you need to reinstall?

In theory, this is possible, but in any case, I advise you to make a backup of the system before doing this, since something can always go wrong during such an update. And if there is a backup, you can always deploy everything back.

So 21H2 won't be there as it will be win11. We'll find out more in 20 minutes.
And in short - LTSC versions are updated only within LTSC. If win11, released in the fall, will have an LTSC edition and M$ will make it possible to upgrade like that, then yes - you can do it. If not, reinstall. But I think that since LTSC builds are the most stable and longest supported versions of the OS, then most likely LTSC will be released in 22 and M$ will make it possible to update to this version.

No. LTSB\LTSC editions do not include an upgrade to a major release. Only security patches.

Windows 10 21H2 LTSC will be. You can update as follows: download the ISO file, unpack it to a local folder, run setup.exe. In this case, it will be possible to save existing files and software. Some software (less than 10%) may stop working or work incorrectly.
I have successfully done this when moving from 1607 LTCB to 1809 LTSC.
