Why do the same sites in different cities occupy radically different positions?
Our brand works throughout Russia. We bought domains like bernd-gorod.ru a long time ago.
The sites are exactly the same, the difference is in the name of the city and prices for services.
50 sites out of 100 are in the top from 1-10 places, 10 are on pages 2-3, 40 domains are not in the search at all.
We thought it might be like domains in the bath, but don’t do anything with them, they don’t rise.
Then they created a site with subdomains and the same situation, some of the cities in the search, some of them in general for 10 pages.
Cities are completely different, both large and small, it doesn’t affect SEO in any way.

It probably depends on the competition. Compare with neighbors and the top in the search results.
Because 1. Google doesn't search... you can type in Google and find out why Google doesn't search...

There are too many factors influencing positions in the search to be able to give an unambiguous answer to your question. Depending on their change, the position also changes.
Here are just a few that come to mind first:
1. Demand for the service in the region of promotion
2. Cost of the service
3. Presence of competitors
4. Quality of content from competitors and their promotional work
5. Demand depending on seasonality
6. Purchasing power in the region
7 As recent events in the world show, the geopolitical situation.
