I use this on several sites.
You can use an unlimited number of fields, you can customize the design through simple CSS to your taste as you like, so far there have been no problems with spam. Copyright is initially set, but it can be removed, no sanctions will follow. The price is 0 rubles.
Of the minuses - it can only be on one page of the site. If you insert on several - you need, respectively, several pages.
And if you want a pop-up form in a separate window, then you will not find such a preparation.
As a last resort, you can use the Google form. I don't know why I don't use it myself. :)
Any form that you find is already separately embedded in a pop-up window. It is not difficult to write it.
