As for your question, it all depends on how you understand the work of a designer. If for you this is a person who can draw beautiful layouts, then, probably, you can not hire a separate person. It will only get worse because you will not understand each other. If for you design is primarily a tool for solving people's problems, then hiring a designer is worth it. The only thing is that it should be a designer, and not just a painter of beautiful pictures.

Or look for those that can be modified to the maximum on their own, and only if there is a sense of style. I once did it on templates, but quickly abandoned it. often came across low-grade, and still have to finish / redo for this or that site, and this is sometimes more exciting than from scratch. Although there are pluses - the client can immediately offer several options.

Depends on the customer. If he needs to do a lot to suit his needs, and he is quite picky about the design, then the services of a designer are needed.
In general, templates are evil. These are viruses, lack of Russification and general understanding of what and how it works. Plus, the craftsmen to put a theme on WordPress are a dime a dozen, this is why there is high competition and mere pennies for work. The owner of the site then runs around in soap with the words "the site is ready, it remains to be configured."
