What to indicate in the game if free music/sounds are used?

I used a couple of sounds, what should be indicated in the game: the author's nickname, where did you get the link? Maybe there is a template?

Usually, they indicate: type of license, year, author and his mail.
Template (if there are no additional conditions from the author):
