What to do when an old customer gives an urgent order?

Is it worth worrying when an old Customer asks to do the work urgently and does not even ask about the cost? Is it worth setting a coefficient for urgency, and which one?

Depending on the complexity of the task, they usually add another 50%.

It is easier and cheaper to retain a customer than to find a new one. If there is a loyal client with repeat orders, this means that they will advise others on you. These should be valued and discarded. At least for urgency, do not take or take less than from the rest.
You will guess the amount for him, he will refuse, think "Here is a beetle" and leave you. And with it, a couple more clients that you didn’t even know about yet will leave. They could appear tomorrow, in a year, etc.
I have not appeared on freelance exchanges for a long time. Word of mouth feeds. Because I do not offend constant. For all the time there were 2 clients with a negative, and then I meaningfully went for it, since I no longer wanted to work with them and the like. I love people who have a brain.
