What is the problem with installing your own module via Magento Connect?

Good day.
I'm dealing with CMS Magento on a local host near Denver: I created my own simple module ("Form" - namespace), packaged it, tried to install it in Magento Connect via upload, but the message " CONNECT ERROR: The 'Form' channel is not installed. Please use the MAGE shell script to install the 'Form' channel. ", and after that the CMS "shut down" and answered all requests with an error 503. What does this mean? What to do, how to fix?
Grateful for any ideas, thanks.

Looks like the package was not packaged correctly. It is expected that the channel will be in the form of a url to your Magento Connect extension. How did you package? How did you set? (step by step with screenshots). Or give a download link. Download and try to install locally. At the same time I will check whether it is correctly packed.

Delete the "maintenance.flag" file in the root of the magenta site. Apparently, when installing the extension, there was a checkmark "Put store on the maintenance mode while installing/upgrading/backup creation". Usually, it is set by default.

Note that the "Channel" field is set to "community". At you for some reason "Form". Hence the error.
