What exactly is money paid for in the application on the Appstore?

3 - for clicking on the link through ads
4 - for downloading the advertised application?

The type of advertising is selected by the advertiser, per impressions or clicks.
The availability of advertising is checked, isAdAvailable();and if there is an advertising company for this user, the playAd();.
For the installation, as far as I know, there is no such thing, after clicking on the commercial, the advertiser can no longer track whether the program was installed or not. Although, it is possible to add an advertising code in the application being installed, which will send information to the advertisers' servers.
