Maps are usually created in an editor made specifically for the game by the game's developers.
Models can be created in almost any of the general purpose 3D editors.
There are many modeling programs available. The undisputed leader is Autodesk Maya, followed by Autodesk 3Ds Max and Cinema 4D. You can also highlight Modo and Blender. The advantage of the latter is that it is free.

Usually games come with an SDK that has everything you need, or converters and plugins for third-party tools. (for example, the Source SDK has a map editor on board and a converter of models from popular formats to its internal one)
Basically, separate tools are used. Graphics are created in specialized programs. There are free ones (Blender, Gimp, Inkscape) and paid ones (Max/Maya, Photoshop, Illustrator). Finished models / textures are exported in formats suitable for the game engine used. The scene itself is assembled from pieces in the scene editor of the engine.
