If you don't want to hire a specialist - read it yourself.
No separate form is required anywhere. One operator? So there is only one agreement. Leave a link in the FAQ or somewhere in the footer and that's it.
By cookies - HZ what you store there. If there is PD, then it is necessary. If not, why not?
And read carefully what exactly is meant by PD. If you have a simple feedback form - you should not bother. You can even shorten "name" to just one field, and that's it. Igor with a phone number - not PD
