Skype is the easiest and cheapest. For grandparents, a laptop is useful, and if you can connect it to a TV, then that's great, but I need to buy a good camera and connect it to a computer and make calls via Skype.

we set up an rmtp server (red5, for example), we drive the stream there using software that transfers this stream to the server - Flash Media Encoder.
This is from the server side.
From the client side - a regular client that digests rmtp (yes, you can even hang a flash player in the browser).
Something like this.
Of course, the quality needs to be chosen so that 3g is enough, but I can immediately say that it will be terrible :)
