Is portforwarding to an external address possible?

Good afternoon.
There is a software router on CentOS with an external address 1.1.1.1
It is necessary that it forwards packets arriving at the address: port 1.1.1.1:5000 to the external address: port of another router 2.2.2.2:5000
Is this even possible?
