I can't figure out how to correctly make a response with span to span?

Hello everyone, in general the problem is this.
I use sharepoint, and I change one page for the questionnaire, but unfortunately I don’t have access to the html file (this is a sharppoint :( )
I add all the html through JS, according to the standard I have 7 questions and they go to 5 tr in which 5 td.
There is an input, which conveys the assessment itself, and there is a label, these are all standard columns, I hig them and add my templates (input and label with for = input so that the animation is displayed) I need my templates to pass this for for standard inputs, but I change the structure I can't, because if in the new label I set for to the standard input, then the animation will not work adequately and the rating will simply not be filled in when clicked
