How to restore access to the account, knowing only the URL of the survey form?

Here is such a situation. Almost 2 years ago, a colleague made a survey through Google Forms on one of the sites. A year later, the person left the company, and everyone safely forgot about this form of survey.
Now I need to access the survey form data again... but none of my colleagues can remember the data for accessing the google account in which this survey form was created. The person who created the corresponding survey could not remember them either.
In this regard, the following questions:
1) Is it possible to somehow find out any identifier of the account under which the survey form was created?
2) Where can I write to Google to ask to restore access to the account, or to remind the email account under which this form was created?
