In addition to noise + blur, working in 16 Bits/Channel, I can also advise you this quick way to get rid of ladders on the gradient: just apply Filter > Filter Gallery -> Brush Strokes -> Spatter to the layer. Leave the Smoothness at 1 and increase the Spray Radius until the stairs are completely gone. This method often helps me out when working in 8 Bits/Channel mode.
