How to process emails if they came to a non-existent address?

There is IIS SMTP and the desire to make the following functionality: the user receives a letter from an address of the form
[email protected]
After the user answers this letter, it is necessary that his answer be processed by the code and if the key and conference ID are in order, then the body of the letter is added to the conference. I googled about IIS modules, but I can’t find a more detailed implementation :(.
Please tell me how to solve this problem? Am I digging in the right direction or is there a more elegant solution? It is advisable not to abandon IIS smtp. Thank you

how about processing emails by email subject? by analogy with any ticket system
