How to organize an express.js application?

I'm starting to understand the NodeJS + ExpressJS + IcedCoffeeScript stack, and the question immediately arose: how best to organize the application code? I mean, how to split folders and all that. This also applies to the client side - I have it on Jade/Stylus/CoffeeScript.
