How to make a WordPress form with LiqPay payment?

The task is to make a form with liqpay payment, and with a radio with different amounts and one radio with any amount (this is for donations). + field for full name, when you click on "Donate" - redirect to the payment page and send information about who donated to the mail.

I tried plugins, I thought to implement it through Woocommerce, but I lack either knowledge or perseverance.

Please tell me how this could be implemented on WP, or are there any examples of a ready-made html form with LiqPay payment.
