Find dll for your version of PHP.
If there is one (most likely it doesn't, because it's empty here: windows.php.net/downloads/pecl/releases/v8js/0.3.0/).
If you want to use pecl - use linux.

1) Unpack the archive into a folder with Open Server (it will not ask you to replace the files because all the files are new, unless of course you installed v8js before).
2) Connect the extension in the PHP config template (write extension = v8js or extension = php_v8js.dll depending on PHP version).
3) Restart Open Server Panel.
