How to formulate a task for revision to programmers?

Situation. The program has a directory of services. Each service can have an additional attribute: whether it is an "initial reception" or "final reception" service.
If a doctor provides a service with the sign "Final appointment", then he chooses how long it is desirable for the patient to come back: after 3 months, six months, a year or 2 years. The business process of the clinic is such that in the middle of this period the doctor must call the patient and inquire about his well-being.
To do this, the doctor's workplace has a "Notebook" menu, where reminders about the need to call are automatically displayed at the right time.
A request was received from the head physician of the clinic: it is necessary to add to the services the ability to set one more sign: "Do not count in statistics." It turns out that they have created the "Call Made" service with a zero price, and the doctor, having called back the patient, marks him with the provision of this service. Managers check the fact of callback using this service. The sign "Do not take into account in statistics" is requested in order not to distort the reporting (average bill, etc.).
Question: how will you convey this request to programmers? We have a bug tracker (TFS) - imagine that you start a task for revision in it.

For good, you should not communicate with programmers and set tasks for them. Your wish for revision should be discussed with an analyst or project manager. Then they formalize your requirement for setting a task for programmers.
