Russia? If you need a tax document, then nothing will come of it. A foreign company needs to be your tax agent and pay contributions to the Social Insurance Fund and withhold personal income tax. You need to apply for an IP, open a foreign currency account, sign a bilingual contract, go through the VC and pay taxes.
If colleagues need more - then any standard form, it still practically does not oblige you to anything.

I took the first few from the SERPs and compiled mine.
