How to customize sale.prder.ajax in Bitrix?

Good time of day. Please tell me how to create an additional block in sale.order.ajax and fill in the order properties there, for example: The name of the block will be called TO WHOM? - and there will be 3 fields type string, and a back and forth button with validation.
Additionally, I would like to argue: I saw a site that is very similar in functionality to checkout, and I would like to copy the js code. Question: Do I need to add php or js code in the template.php file or somewhere else?
