In principle, such a tool as Google Forms is enough, but there are some peculiarities.
1. Google Forms - must be viewed through a browser
2. In Google Forms, it is not possible to return a response when the submission has already taken place.
3. Google Forms does not have a pre-registration mechanism so that all actions are performed with a registered participant.
