How to correctly implement a contact form on the site?

Good afternoon.
There is a page with blocks of services (about 10 blocks), where each one has an "Order" button. It is required that when the button is pressed, a form pops up where a person enters his contact details and sends them to the mail , and the recipient has an understanding of which form the application is from . Plugin with forms Contanct Form 7.
The problem is that if you bind one form template to all buttons, it is not clear which service the user has chosen.
You can bind several forms (10 forms + more will be added), but isn't this bad manners? Maybe there is a more elegant solution? Tell me please

without a plugin .. standalone... each block form has its own php file
