How to change the testimonials form in the Strong Testimonials plugin?

For the second day I can not figure out how to edit the feedback form in this plugin.
I looked at examples of shortcodes on the developer's website. I learned how to make a certain number of reviews per page, etc., etc., but how can I change the format of reviews? for example, this demos.wpmission.com/strong-testimonials/examples/a...
When you enter the shortcode, which is written below, the feedback form still remains as it was (screen 1).
ps I also found the Handsome Testimonials plugin, where you can change the form of reviews as you wish, BUT there is no block for writing it (only through the admin panel).
