But if you really want to do this in Photoshop, then you need to write your name with your hand (it is desirable that the same person did it and the handwriting matches), take a picture, clean up the insertion space and photoshop it to the picture, spending time on final retouching of the patch. But this will also take longer than handwriting and plus the handwriting will most likely be different.
PS It is theoretically possible to spell out and make up a name, as Evgeny advises, but it is also very labor-intensive.
