How to ask for a letter of recommendation?

When can you ask for a letter of recommendation? And who? At the team lead, technical director or Deer? Or from everyone? From whom are they most valuable? Can be beaten in the door of the alma mater to the most deserving teachers? In what format? Is it printed with company seals or is it just enough of the letter itself in electronic form with the coordinates of the recommender? What if foreigners ask for letters of recommendation? Immediately ask that they write in English (we are programmers, everyone knows English)? Or to translators?

I will state only my own opinion:
- of the above, a letter from the technical director would be most interesting;
- on printed paper;
- from a teacher - only if he taught you several special disciplines and / or course projects, excellent - if a graduation project; moreover, in this case, the letter would be most interested in assessing your independence of analysis and the chosen solutions (to a lesser extent - technical study, since it is better assessed by practitioners); the eminence of the teacher is also not the most important thing in this matter - if this coincides with the first requirement, then it is wonderful, and if not, then the first is more important;
- in Russian (translation into foreign languages ​​is your concern, asking in a foreign language is some familiarity in relation to the author of the letter, unless he himself offers it); + there are notarized translation services, if you really need it, however, I believe, just a beautifully designed translation, along with the original with seals, will be enough.
