How do I fill in my home address on oDesk?

There are 3 fields in the questionnaire: 2 fields are address and one is city. So, how can I fill it all out correctly in English, can anyone help?
Here are the initial data:
Zaporozhye region. Kuibyshevsky district, town. Kuibyshevo. Krupskaya street, house XX. (PS: I live in a private house). Thank you.

City - Kuybyshevo
Address - Krupskoy str., XX
If there is no index field, then you need to insert it at the beginning of Address.
