Application for iPhone, Android and Blackberry, which is installed under the guise of "Testapp". Once established, it has access to phone calls, sms, addresses.

An application that installs itself on iOS, without user intervention, without Jailbreak? I don't think this is possible. So far, there have been no precedents, as far as I know.
An iOS app that has access to calls, messages, and addresses? This is quite possible, even without Jailbreak. Information on calls and messages can be obtained using undocumented APIs from Core Telephony, for example.
