Application monitoring in Zabbix 3.2?

Hello.
There was a need to monitor the status of some applications, incl. self-written. I intend to do this through Zabbix 3.2. How can I get and send information to Zabbix Server that the application is (not) running and how much resources it consumes?
Thank you.

@HeroFromEarth

Zabbix agent has been able to do this for a long time
Everything is indicated in the documentation
Examples:
proc_info[iexplore.exe,wkset,sum] - to get the total amount of physical memory allocated for all Internet Explorer processes proc_info
[iexplore.exe,pf,avg] - to get the average number of page faults for Internet Explorer processes
that for the correct operation of this item on a 64-bit system, a 64-bit Zabbix agent is required.
Note: All io_*, gdiobj, and userobj attributes are available only on Windows 2000 and later versions of Windows, not on Windows NT 4.0.
