A system for filling data on an android device for printing according to a template and adding to the database?

Good day to all!
I am looking for some ready-made or semi-finished (with the possibility of editing for this case) solution for such a task:

1. the client comes to the company, approaches the Android tablet, which displays a data entry form (a simple format with a designation and an input field);
   
2. (additionally) it would be nice if the system could understand that the user's data is already in the system, for example, by the phone number when the user entered it, and insert already known information into the form;
   
3. after entering all the data, he presses the OK button (the system checks that all fields are filled in), after which the data from the form is sent to 2 places: to a database (for example, Access) and to print according to a certain template (no need to save the file).
   

The system must work without the Internet. Both paid and free options are considered.
