WYSIWYG form builder that you can embed on your website

Tell me, is there any relatively lightweight library for PHP that allows you to organize a wysiwyg builder of simple forms (text fields, drop-down lists, etc.) on your site , and after the user finishes, it gives out the form structure ( field names, styles, etc.) so that they can then be saved somewhere and processed later?

In terms of wysiwyg functionality, this thing - www.jotform.com/ - seems quite attractive to me, but, unfortunately, they do not give out the code (they only allow you to build a form on their site). And I need it in the form of a code, for embedding in my system.

They give JotForm the code too , but it's not cheap.

Probably not very helpful, but Drupal has a form_builder module (there is a link to the demo on the right).
