Why is the volume slider on the speakers (in headphones) junk?

Very often I notice that on audio systems the hardware increase / decrease slider is junk. It got to the point that it is very difficult to set up a uniform clear sound on both sides of the speakers. Why is this happening, can it be fixed and how to avoid it?
