So you answered your own question. The plug-in in the browser allows traffic through an external proxy and does not affect the flash settings and name resolution, hence the DNS and Flash leaks, in addition, proxy ports are open on the server that uses firgate and it is hosted.
When using a VPN, all traffic, incl. name resolution and Flash goes through the VPN.
