Why is an unremarkable site always in the top 1 for a keyword?

For example, for the keyword turnkey website development , Mega Group is always in the top. Although such firms, a million if not more.
And all of them were indexed in Google, and many of them chug and chug with optimizations, but they just can't get into the top for the keyword "turnkey website development".
