Why is an offset filter applied to the duplicated layer when duplicating a layer on a new document?

Photoshop fucked up!!!
Tell me why when duplicating a layer on a new document, an offset filter is applied to the duplicated layer? I've googled all over and found nothing. Can you tell me where it can be turned off?

And how do you "duplicate" to a new document? Not copy/paste by accident?
