Which bugtracker to choose?

We need a simple widget on the site to receive bug reports from users. I mean the finished product, I came across such a couple of times - they looked like an ordinary online consultant, some are simpler, some are more difficult with task ratings and other bells and whistles.
Advise ready solution. Do not advise widgets that work like screenshots.

It all depends on the methodology used in the company. We use Jira, Kanban methodology
