Koterov PHP5 in the pod. The book is a bit outdated, but still the best one I've seen. Unlike many other books, it does not aim to make a PHP coder out of a PHP reader. While reading, it would be nice to set the task of making a business card site for yourself or your friends with a simple admin panel that can be improved as you read. It is pointless to give further advice now, everything will depend on the results and on your preferences.

I recommend to start by learning some kind of framework. Yii or Symfony 2.
